The FORMED website requires a one-time sign-up for parishioners.
To sign up, click FORMED Sign Up (or click Sign Up on the FORMED website) and perform the following:
- Search for our parish using St Francis of Assisi Wichita KS, then select our parish from the search results
- Provide your name and an e-mail address to use during sign in
